Sign your waiver on your phone
This is the consent form for the tattoo or piercing shop whose QR code you scanned. It runs on waivers.tattoo, which the shop uses to take and keep its consent records. Turn JavaScript on to fill it in, or ask the shop for a paper copy.
Hearing from the shop by text or email
Above the signature the form asks for your mobile number, and under the box that accepts the terms there are two more boxes, both optional and both unticked. You may tick either, both or neither and still be tattooed or pierced — saying yes is not a condition of getting service. They read:
- “Yes, [the shop] can text or email me about my aftercare and how my healing is going. Optional. Msg & data rates may apply; reply STOP any time.”
- “Yes, [the shop] can text or email me its news, offers and events. Optional. Msg & data rates may apply; reply STOP any time.”
Texts are sent by waivers.tattoo on the shop’s behalf and every one names the shop it is from. How often depends on the shop. Message and data rates may apply. Reply STOP to any message to stop them, or HELP for help.
Mobile phone numbers and text-message opt-in consent are never shared with third parties or affiliates for marketing or promotional purposes.
